MSIC Online Application Form
Welcome to Veritas Engineering Pty Ltd's on-line pre-printed application form site.
This is a secure site designed to help customers correctly complete an MSIC application form online. Applying for an MSIC is a 3-step process and takes approximately 10 - 12 minutes to complete:

The standard price for an MSIC starts at $234 + GST. This price includes processing of your application in person at our Perth CBD office and delivery of your card to your listed postal address by Registered Post. If you cannot apply in person at our Perth office, you have the option to apply at any one of over 4,000 Australia Post outlets. To use this option, a $18 processing fee will apply.
Application notes:
Step 1 - Takes approximately 10 - 12 minutes to complete online. Once you have completed your online form, we will ask you to print a copy of the form.
If your employer is registered with us, they will be able to assist us to verify your "operational need". If your employer is not registered with us, you will need to provide us with satisfactory evidence of a demonstrated need for an MSIC. Details on the evidence required and where to send this information to, will be included in the printed application pack. Please contact us if you are unsure whether your employer is registered with us.
Step 2 - When you submit your MSIC application form, you will need to verify your identity through the provision of identification documents. A complete listing of acceptable documents are provided in the Downloads section of this website; a reminder listing will also be provided in the printed application pack.
Step 3 - You have three options to submit your application form and passport photo:
- Option 1 - at the Veritas Engineering Perth CBD office (to reduce wait time, please phone us on 08 9226 0711 or 1300 651 955 to make an appointment to submit your application and have your passport photo taken).
- Option 2 - at your nearest Australia Post outlet (some restrictions may apply for usage of some forms of identification). To find your nearest Australia Post branch please use the Australia Post - Post Office Locator.
- Option 3 - through a Veritas Engineering representative at your place of employment (if coordinated by your employer). Please contact us for queries.
Passport photos are taken free of charge when applying via our Perth CBD office
On 29 January 2010 the Minister for Infrastructure, Transport, Regional Development and Local Government, Minister Albanese, announced changes to the MSIC scheme.
